Case Load: 12-21 Avg. Daily Census: 160 Schedule: What is required? M-F every other weekend days 8-4:30p 30 minute lunch M-F, Weekends, day/night shift, holidays, rotating, lunch :30? UR software: InterQual & Epic Milliman MIDAS CERME Book Other Discharge Planning Software: ECIN Allscripts Client environment questions: How many staff CMs in department/ED? 71 What floors will the CMs primarily cover? (crossover on multiple units) Float Daily Rounds, when? 10:00 and 3pm Will our CM lead the rounds? yes Average Case Load of the units? 12-21 Can you explain the environment and what our Case managers can expect once they start working? Mixed Case population, hospital has Dyad model of care Physician and Case Manager Do you have social workers for discharge planning, or will the CMs complete UR and discharge planning?
