LifeLong Medical Care is looking for a part-time Nurse Practitioner
or Physician Assistant committed to providing high-quality
outpatient primary care and street health at our Medical Respite
Center. The ideal candidate must be dedicated to building a
collaborative team model of care experience working with the
under-served. Under the direction of a supervising physician,
provides general medical service at our Medical Respite in West
Oakland. Lifelong's Respite program is a unique program that
provides housing and semi-acute medical and nursing care in a 27
bed facility. We take patients experiencing homelessness who are
too ill or frail to recover from a physical illness or injury on
the streets but are not ill enough to be in a hospital. This
position functions as a critical member of the care team, ensuring
team communication, quality care, and patient satisfaction. At

bargaining agreement (CBA), and an employee in this position must
remain a member in good standing of UAPD, as defined in the CBA.

community, LifeLong serves over 66,000 patients in the East Bay
Area's socio-economically underserved regions. We provide medical,
dental, behavioral health, and school-based services at over 15
primary care sites across Oakland, Berkeley, and West Contra Costa
County. From opening a residential respite program for homeless
persons discharged from the hospital to offering free COVID testing

$123,000-146,000/year and pro-rated depending on years since
licensed, sign-on bonus, and excellent benefits: medical, dental,
vision, Flexible Spending Accounts, dependent and domestic partner
coverage, 403(b) retirement savings plan, and loan repayment
programs. LifeLong is a qualified site for federal and state loan
repayment programs (HPSA score is 21). Acceptance rates may vary
year to year based on funding levels and the number of applicants.
