dedicated and passionate Physical Therapist/PTA to join our team at

Responsibilities: Develop and implement individualized treatment
plans for patients Conduct home visits and provide hands-on therapy

Collaborate with other healthcare professionals to ensure
comprehensive care Document patient progress and communicate with
the healthcare team Requirements: Valid Physical Therapist license
in California Minimum of 1 year of experience in home health or a
similar setting Strong communication and interpersonal skills
Ability to work independently and manage time effectively Passion
