Job Description:
The Spiritual Chaplain/Counselor is a compassionate professional
responsible for providing emotional and spiritual support to
patients, families, and caregivers facing the challenges of
terminal illness or recovery at home. This role involves
independent management of a caseload, utilizing advanced
counseling, assessment, and decision-making skills to address the
unique needs of each individual or family. The Chaplain ensures
that the emotional and spiritual needs of patients are met while
collaborating with interdisciplinary teams to promote holistic
care. Our Hospice Care Mission is to provide compassionate and
quality end-of-life care and grief support to patients and their
families. We provide emotional comfort during the last phase of
life, ensuring dignity and respect for all involved. Our Home
Health Care Mission is to assist patients in achieving the highest
